Interdental brushes (IDBs), also referred to as interproximal toothbrushes, are at -home toothbrushes that play a crucial role in oral hygiene regimens. In contrast to traditional toothbrushes, these often feature an extended, thin design with flexible bristles that allow them to adapt to different interdental gaps. Therefore, to meet the needs of every person, these adaptable instruments come in an enormous range of sizes and shapes.

What are several types of interdental brushes?

People may find it challenging to select an interproximal brush because there are so many options available. In 2107, Larsen et al. conducted a crossover clinical trial  to evaluate the efficacy of conical and cylindrical interdental brushes in removing interproximal biofilm. In contrast to the previous studies concluding their better adaptation, this research concluded that conically shaped brushes performed lower in the testing when it came to proximal-lingual surfaces, and there was no appreciable change for the remaining surfaces of proximal and proximal-vestibular surfaces. 

It was also discovered that, in comparison to straight interdental brushes, waist-shaped interdental brush heads removed noticeably more plaque biofilm. This can be explained by the better cleaning performance of waist-shaped brushes on the adjoining buccal and lingual tooth surfaces.

Patients who have limited manual dexterity may find it helpful to use a brush with a wider or modified handle. For those who need to clean the interdental areas in the posterior region, a flexible or L-shaped brush can be the best option. The brush should not require force to operate, and its size should allow it to fit snugly in the interproximal region.

Patients may require different sizes for distinct areas. While utilising them may initially seem laborious or complex, it doesn’t take long to use them and establish a routine habit with them. It is therefore essential to guide the patient in selecting and using the appropriate size of the interproximal brush. On the global market, numerous manufacturers such as GUM®, TePe®, Interprox®, Pico Jenner®, etc. provide diameters ranging from 0.4 mm to 2.2 mm. (These diameters might be coded by specific colours)

It is also essential to take the material of the interdental brush into account. It has been observed that the metal wire positioned in the middle of the IDBs may irritate patients with sensitive root surfaces. Modern inventions like rubber interdental brushes and picks could be a useful replacement for more conventional IDBs. They offer the benefits of better patient compliance and acceptability in terms of comfort and willingness to purchase the product and have been shown to be equally effective as conventional or metal-core interdental brushes.

Who is a suitable candidate for interproximal brushes?

Patients whose teeth are too sensitive for a standard toothbrush should use an interdental brush to completely remove any residual or lodged food materials. IDB is especially important for those who have open interdental gaps, crowns, braces, implants, gum disease, foul breath, or exposed furcation areas. It is imperative to emphasise that interdental spaces that harbour high plaque buildup, which cannot be effectively cleaned by toothbrushes or floss, necessitate the use of an IDB. 

It is crucial to remember that interdental brushing can be an adjunct but can never replace the conventional toothbrush. 

How should an interdental brush be chosen?

The interdental spaces make colour identification easier. This helps in determining the most appropriate type and location for toothbrush use.
The brush should not be forced; rather, it should fit properly between the teeth or gaps. The ultrafine interdental brush can be used to start with this practice. Nevertheless, it is vital to take into account the following:

  • Well-balanced design for a secure grip.
  • A compact and comfortable head that makes it possible to access even the hardest-to-reach places.
  • Gentle bristles to prevent harm to gums and enamel.

What is the correct way of using the IDBs?

Despite being a supportive tool for interdental cleaning, one must use them correctly, as indicated by the following guidelines:

  • Place the interdental brush tip where you want it to reach between your teeth. Then, using a gentle probe, find the best angle to begin softly inserting the brush without using force. Please be informed that as you practice, you’ll find that some angles work better than others.
  • To achieve better cleaning and more control, rotate the brush tip to a more horizontal position before inserting it too deeply.
  • It is recommended to use interdental brushes with straight handles in anterior areas. 
  • If cleaning posterior areas such as molars or premolars, use an interdental brush with an angled head or bend the brush at 45° or 90° as required.
  • Gently and carefully clean the area between your teeth till you get to the other side. If you feel resistance or pain, it is advisable to halt and carefully try again.
  • Brush your teeth and gums in a back-and-forth motion to ensure thorough cleaning. This usually suffices to get rid of any stuff that has accumulated. Make sure to thoroughly rinse the brush after using it.
  • The interproximal brush should always be kept in position.

These IDBs shouldn’t be forced into place, and if they are bent, they shouldn’t be straightened either for fear of breaking their heads. If the brush head breaks and remains in between the teeth, it can be removed with another interdental brush. 

Can IDBs be utilised on a regular basis? If so, what is their maximum duration? 

The interdental brushes are intended for daily usage and should be used in conjunction with a toothbrush as recommended. These are also very handy to carry around and can be employed in circumstances when a traditional brush is not an option. These usually have a thirty-day shelf life. If there is significant filament wear, it is highly suggestive that the brush needs to be replaced.

Which is better, interdental brushing or flossing?

It is recommended when there is room between teeth to use the tiny IDBs—instead of floss.” On the other hand, flossing could be a good substitute if IDBs are unavailable. 

However, when deciding the type of interdental cleaning to recommend to a patient, numerous considerations must be made. These include the patient’s compliance, dexterity, gum disease severity, and the size of the spaces between their teeth.

Research has indicated that interdental brushes are superior to floss in terms of minimising gum inflammation and plaque in patients with gum disease and adequate space between teeth.

IDBs serve as vital interdental cleaning tools for preserving the best possible oral health, particularly for people with certain dental problems such as gum disease, open interdental gaps, and dental treatment like implants and braces. These brushes successfully clean areas where conventional toothbrushes and floss might not reach, avoiding the accumulation of biofilm and plaque, which might worsen the existing dental issues. Interdental brushes ensure a cleaner, fresher mouth and contribute to overall oral well-being by enhancing gum and dental health and improving oral hygiene routines daily. Making their use a priority may result in long-lasting oral benefits and a healthier smile.
