Why does the brain have no pain receptors?

When people talk about a “splitting headache,” it’s easy to imagine the pain coming directly from the brain. But here’s an interesting fact: the pain receptors in brain itself don’t exist. Simply speaking, the brain has no pain receptors. That means your brain can’t actually feel pain the way your skin or muscles do. Surprising, right? Let’s understand why that is, how pain is really sensed, and what it means for common problems like headaches.

If there are no pain receptors in brain, how do we still feel pain?

Dr Joy Mounica, Consultant Neurologist, Renova Hospitals, Langar House, Hyderabad, says, “The reason lies in how our body’s ‘pain alarm system’ works. Pain starts when pain receptors, also called nociceptors, detect injury or pressure in body tissues like skin, muscles, or organs. These receptors send warning messages to the spinal cord, which passes them to the brain. The brain then interprets these messages as pain.”

“However, the pain receptors in brain itself don’t exist. The brain tissue, the gray and white matter that helps you think, feel, and move, doesn’t have these nociceptors. So, even though the brain processes pain, it doesn’t sense it,” explains Dr Mounica. 

This is why neurosurgeons can perform certain brain surgeries while a patient is awake, and they won’t feel pain, only pressure or vibration when the skull or scalp is touched. In short, your brain is like the control centre that receives and interprets pain signals from all over the body, but the centre itself is “pain-free.”

If the brain has no pain receptors, what causes headaches then?

This is one of the most common questions people ask. If the brain has no pain receptors, why does the head hurt so much during a migraine or tension headache?

Here’s the answer: headaches don’t come from the brain tissue; they come from the structures around it. The meninges (the protective layers covering the brain), blood vessels, scalp, and neck muscles all have pain receptors in brain coverings. When these tissues swell, tighten, or become irritated, the receptors send signals to the brain, creating the sensation of head pain.

For example:

  • In migraine, blood vessel changes around the brain trigger pain signals. Migraine pains may also alter your vision and cause kaleidoscope vision
  • In tension headaches, tight scalp or neck muscles send pain messages.
  • In sinus headaches, inflammation near the forehead and eyes activates pain receptors in the surrounding tissues.

This understanding also explains headache pathophysiology, the biological process behind why we “feel” a headache even though the brain itself is pain-free.

Why is it helpful that there are no pain receptors in brain tissue?

It might sound odd, but the absence of pain receptors in brain is actually beneficial. Here’s why:

  • Protection from constant pain: The brain constantly sends and receives signals. If it had pain receptors, normal activities like thinking or blood flow could trigger continuous discomfort.
  • Safe for surgery: During brain surgeries, doctors can safely work on the brain tissue itself while patients remain awake, helping doctors test movement or speech functions.
  • Focus on vital signals: The pain system works best when it alerts us to external or internal injury. Since the brain is well-protected by the skull, cerebrospinal fluid, and meninges, it doesn’t need to detect pain directly.

However, when the surrounding structures are affected, like in swelling, infection, or trauma, the pain receptors in brain coverings step in, warning us that something nearby is wrong. So, even though the brain can’t feel pain itself, it helps us experience and respond to pain elsewhere in the body.

Understanding that the brain has no pain receptors doesn’t make your headache any less real, but it helps you know where that pain is truly coming from. The pain receptors in brain may not exist, but your brain’s ability to understand and respond to pain is one of the most remarkable things about how our body works.

So the next time you feel a headache coming on, take care of your body, rest your mind, and appreciate the silent, tireless organ that feels everything, yet never feels pain itself.
