Midlife crisis is as real as any other mental issue. Many psychologists believe that a midlife crisis is nothing but an emotional response to changes in the environment, newfound freedom, an abundance of time, and loneliness. It affects some more than others. Here’s how it affects people and what to do about it.

Is midlife crisis real?

Yes, midlife crisis is a real phenomenon though different in intensity and form among people. Midlife crisis refers to the emotional turmoil or self-reflection, some people undergo during middle adulthood, roughly between the ages of 40 and 60. People with midlife crises are dissatisfied with their lives, question choices, or aspire for major life changes to reinstate a sense of purpose or fulfilment.

However, not everybody goes through this crisis. Some psychologists assume what most people call a ‘midlife crisis’ is a response to the pressure or challenges of life transitions, like ageing, a shift in career patterns, or family dynamics rather than an inevitable phase of life. Importantly, this midlife crisis is also of varying intensity from one person to the next. Some people pass through this stage without much disturbance, while others make drastic changes by changing careers or relationships.

There is also much debate in psychological research about how wide and severe the phenomenon is. Some even propose that it is less common than popularly perceived, and perhaps more of a cultural concept than a human experience.

How do hormonal changes affect mood in midlife?

Dr Madhu Vamsi, MBBS, MD (Psychiatry), Consultant Psychiatrist, Hyderabad, Telangana says, “As we age, our mental health needs change. Here are some common concerns I see in my practice: anxiety, depression, midlife crisis, memory loss, relationship issues, chronic stress, and trauma.” 

Changes in hormonal activity in midlife affect mood, particularly when female hormones lower during menopause or male hormones like testosterone decrease. Generally, such changes manifest themselves as increased irritation, anxiety, and depression, although effects may vary from person to person.

Changes in women

Menopause: Most women experience a slow drop in the levels of estrogen and progesterone, which defines perimenopause. This results in a lot of mood swings, from irritability to sadness, as these hormones play an important role in mood regulation.

Depression: Many women are at higher risk for depression during the perimenopause years because estrogen causes a precipitous drop, affecting levels of the ‘feel-good’ neurotransmitter.

Anxiety and Stress: Estrogen also plays a role in how the brain responds to stress. Lower levels may make women feel more anxious or overwhelmed by stress.

Sleep Disturbances: Hot flashes and night sweats, in particular, which characterise menopause, often disrupt sleep, and may exacerbate mood and contribute to feelings of fatigue and irritability.

In men

Testosterone Decline: Men gradually face a decline in their testosterone production, which is known as ‘andropause’. Although testosterone decreases less sharply than in women, the reduction can have mood-related implications, such as:

  • Irascibility and Low Mood: Limited levels of testosterone may cause irritability, depression-like symptoms, fatigue, etc.
  • Lack of Interest and Energy: The loss of testosterone can also make him lazy and lack the motivation he needs to deal with situations and people.
  • Stress and Ageing: The middle-aged person will also have more responsibilities at work and home which will increase the levels of stress and might result in mood changes.

Common outcomes in both sexes

Cognitive Decline: Individuals of both sexes suffer from slower processing or forgetfulness midlife, which sometimes increases frustration or mood swings.

Social Factors: Changes in the home, such as children leaving their parent’s house or a career change, are also a source of stress and contribute to mood change which has previously been influenced by hormonal changes.

What are healthy coping strategies for midlife mental health challenges?

Dr Vamsi says, “Making small changes can add up to make a big difference in our mental health! Here are some lifestyle tweaks to try: exercise regularly, eat a balanced diet, get enough sleep, practice mindfulness, stay connected with loved ones, engage in activities you love, and limit screen time.”

Dr Vamsi adds, “A healthy way of dealing with issues during midlife involves a balanced lifestyle such as regular exercise, diet, and rest. Exercise is helpful because it triggers the secretion of endorphins, which help improve mood and eliminate stress. In addition, other mindfulness practices such as meditation or yoga could prove helpful in warding off anxiety and achieving emotional balance. Even support from friends or support groups, and often from the family, can also be of utmost importance to help maintain connectedness and sharing feelings in the reduction of that sense of isolation sometimes accompanying midlife transition.”

Professional assistance, for instance, therapy or counselling can also be effective in managing midlife mental health concerns. It equips people to shift unhealthy thinking into more positive and productive ways of dealing with distress. Unhealthy attitudes such as alcoholism or overworking too much also have to be mitigated to cause less emotional toll. Turning this period into a time of personal growth and discovery, therefore, may make this rather crucial passage of life more satisfying and resilient.
