Why does weight loss in fever happen even if you’re not trying to lose weight?

Dr Ubaid ur Rahman, a General Physician, says, “If you’ve just recovered from a fever and your clothes feel looser, you’re not imagining it. Weight loss in fever is quite common, even when you’re not trying to shed any kilos. Here’s why: when you have a fever, your body temperature rises (around  100.4° F or 38° C). This makes your body work harder to fight the infection.” As a result:

  • Your metabolism speeds up, burning more calories than usual.
  • You sweat more, which causes water loss.
  • You may eat less because fever often kills your appetite.
  • Your body may break down muscle for energy if you’re not eating enough.

All these factors may lead to weight loss in fever, even over just a few days. Dr Sunil K. Khetarpal, MBBS, MHA, Director, AHPI (Association of Healthcare Providers India), Delhi, explains, “ Fever is a symptom of many other diseases, like tuberculosis, which may lead to weight loss, fatigue, loss of appetite, etc.” Weight loss may be associated with fever, abdominal discomfort or pain, diarrhoea, or even constipation.

Is the weight loss in fever mostly water, fat, or muscle, and why does that matter?

Many people think they’ve lost fat during a fever. But in most cases, that’s not true. Let’s break it down:

  • Water loss is common due to sweating, especially if you have a high fever or night sweats.
  • Muscle loss may happen because the body starts using muscle for energy when you aren’t eating well.
  • Fat loss may happen, too, but it’s usually not the main reason.

This is important to know because losing muscle is not healthy, especially if the fever lasts many days. Muscles help you stay strong and support your immune system. Also, losing a lot of water (dehydration) can make you feel weak, dizzy, and tired.

So while weight loss in fever might seem like a quick fix to lose a few kilos, it’s not a healthy or lasting way to lose weight. In fact, it can make your recovery slower.

When should you worry and see a doctor?

Some amount of weight loss in fever is normal. But here’s when you should be concerned:

  • If your weight keeps dropping even after your fever is gone.
  • If you’ve lost more than 5% of your body weight in a short time.
  • If you’re feeling very weak, tired all the time, or losing your appetite completely.
  • If you also have night sweats, a cough, or feel breathless.
  • If you had a low-grade fever that lasted for weeks.

These can be signs of something more serious, like tuberculosis, thyroid issues, or gut infections, which are common in India. In such cases, it’s important to see a doctor. Don’t try to fix it yourself with home remedies. Rapid weight loss while sick can be dangerous if left unchecked.

Weight loss in fever is your body’s way of coping with stress and infection. While it’s common, it can sometimes be a red flag, especially if it continues after recovery. Remember, your health matters. And taking care of yourself after a fever is just as important as treating the fever itself. Let your recovery be slow, steady, and safe.
