Q. What do menstrual cramps mean and are they normal?

Menstrual cramps are a feeling which you’ll get an hour or 2 hours before you start bleeding and then after, or during the bleeding. It is a sensation of pain in the lower abdominal and a little bit in your vaginal area, in your legs and in your thighs. It is a feeling of pain and heaviness. This is a menstrual cramp.

In medical science menstrual cramps is known as dysmenorrhea. Yes, it’s normal only if there’s other issues in the uterus, like endometriosis or ovarian cysts or any fibroid in the uterus. If you feel excessive pain, you can visit a gynae and they might suggest a regular ultrasound or a scan.

Q. What are the causes of these painful cramps?

In most of the cases, this will be normal. So, some possible reasons could be: first, endometriosis. Second, ovarian cyst or any condition, or chocolate cyst or cysts in the ovaries. This condition is adenomyosis. However, one needs to go through thorough medical investigations before reaching any conclusion. Young girls should note that menstrual cramps are not always normal, these might be the symptoms of underlying diseases. Hence, visit a gynaecologist.

Q. Are there any home remedies to relieve the menstrual cramps?

Generally, I do not advise home based remedies. If somebody wants to avoid any painkiller or any antispasmodic medicine, then they have the option of heat or pain relief gels. However, these are not effective as they simply remain on the skin or surface level. Hot water bottle would relieve lower back pain but it takes a lot of time.

Q. When to be concerned and see a doctor?

If menstrual cramps impact your quality of life and you’re not able to maintain the daily routine then it’s time to see a doctor. Also, one should not have a phobia of periods or menstruation. Some take injections during those days since oral medications are not effective for them. In all these cases, seek medical advice.

Q. How do gynaecologists treat these menstrual cramps?

First, a general basic ultrasound scan to check if there’s any pathology. If nothing is found, then it’s a normal menstrual cramp and we write regular medicines like antispasmodic or painkillers. These tablets are for once a day starting on the first day, as this is where the pain starts. We also prescribe Drotaverine for the uterus and this gives you relief for 7 to 8 hours. These medicines or painkillers shouldn’t be taken without the prescription of a doctor. As always, safety and right knowledge is the key.
