Dr Choudhary: Liposuction is medically safe for appropriate candidates when performed under proper conditions; however, it is crucial to understand that it is still a surgical procedure with inherent risks, and outcomes depend on both patient factors and the skill of the surgical team.

Q: What pre-surgery tests or assessments are usually required to ensure patient safety?

Dr Choudhary: Just like any other surgery, liposuction also necessitates certain pre-operative evaluations. These tests help ensure safety and smooth recovery. A comprehensive medical history, along with clinical examination and BMI calculation, may be supported with some of the following assessments:

  • A complete Blood count helps check for anaemia, infection, or immune-related issues
  • Coagulation tests, such as PT/INR, aPTT, measure clotting times. This may help reduce bleeding or clotting risks.
  • LFTs and KFTs: These help evaluate how well the body can process medications and eliminate fluids. Any abnormalities indicate adjustments in the surgical approach.
  • Blood glucose examination helps identify diabetes or metabolic issues – a crucial complication for postsurgical healing
  • Electrolyte panel checks
  • For patients above 40 years ECG may be advised. They may help assess hearth health and anaesthesia readiness.
  • If indicated, chest x-rays may be advised.
  • Most significantly, screening for HIV and Hepatitis B and C ensures both patient safety and adequate peri-operative precautions.

Q: Are there certain health conditions that make liposuction riskier?

Dr Choudhary: Yes, there are certain health conditions that may increase the risks associated with liposuction. As a result, the procedure may not be suitable or necessitate specific precautions for some. Anyone undergoing liposuction should be systemically healthy without any chronic illness, as it could complicate the surgical outcome.

A candidate for liposuction should be in overall good health, without chronic conditions that could complicate surgery or recovery. Health conditions that could enhance the liposuction related risk may include:

  • Severe cardiovascular conditions, such as a recent heart attack, unstable angina, heart failure, or significant arrhythmias, can elevate surgical and anesthetic risk.
  • Blood clotting abnormalities (e.g., hemophilia, those on anticoagulants) can increase bleeding risk.
  • Poorly controlled blood sugar levels may impair wound healing and increase infection risk.
  • Chronic obstructive pulmonary disease (COPD), pulmonary edema, or recent pneumonia make anesthesia and recovery much more difficult.
  • Any ongoing infection, especially near the planned surgical site, must be resolved before surgery.
  • Impaired hepatic or renal function may significantly alter drug metabolism and fluid balance.

Dr Choudhary: While the majority of patients are aware of the usual adverse effects, many fail to consider the significant risks of liposuction, both immediate and long-term. These primarily consist of:

Short-Term Risks:

  • Bruising, swelling, and discomfort can persist for several weeks after the procedure.
  • Fluid buildup (seroma) beneath the skin is common and sometimes needs to be drained with a needle.
  • Temporary numbness is frequent, but in some cases, sensation changes may be permanent.
  • Infection, although rare, can become serious if not addressed promptly
  • Blood clots, such as deep vein thrombosis (DVT), can occur and are potentially life-threatening.

Long-Term Risks:

  • Persistent contour irregularities or skin depressions may become permanent and require further corrective procedures.
  • Skin may remain loose or saggy, especially if large amounts of fat are removed or if the skin has poor elasticity.
  • Long-term or permanent nerve damage leading to persistent numbness or chronic pain, can develop.
  • Fat embolism though rare, is potentially fatal.
  • Fat may be redistributed to other areas of the body, leading to abnormal fat deposits if weight is gained after surgery.
  • Damage to deeper structures like nerves, blood vessels, or organs is rare but may have life-long consequences.

Q: How has technology changed liposuction in the past decade? Is it safer now?

Dr Choudhary: Modern liposuction has significantly evolved. These improvements have made the procedure safer, more efficient and patient friendly. Energy-assisted procedures, such as Ultrasound-assisted (UAL, e.g., VASER), laser-assisted (LAL), radiofrequency-assisted (RFAL), and water-assisted (WAL) liposuction, now allow more precise targeting of fat and often promote better skin tightening. Power-Assisted Liposuction that employs vibrating cannulas has made fat removal more efficient and less physically demanding.

Additionally, the introduction of smaller cannulas, improved fluid management systems, and less invasive incision techniques has led to reduced scarring, swelling, bleeding, and downtime for patients – allow smoother recovery. Adding to this, the integration of intraoperative imaging and artificial intelligence has enabled surgeons to navigate tissue more precisely, reduce contour irregularities, and customise treatment for individual patients.

Most significantly, the overall risk of complications—particularly serious ones—has been further minimized when procedures are performed by trained specialists using current-generation technologies of surgery and anaesthesia.
