Food Anxiety or ARFID: Beyond Fussy Eating

Just In

When a child doesn’t eat well, many parents generalize it and disregard it as “picky eating” or the child being too adamant but have you ever wondered it could be more than that? Avoidant/Restrictive Food Intake Disorder (ARFID) is an eating disorder where a child eats too little or avoids certain foods; not because of weight concerns, but due to fear, discomfort, sensory issues, or lack of interest in food. Often taken too lightly, it can have a serious effect on child’s growth, nutrition, and daily life, making mealtimes stressful for both the child and family.

In a discussion with THIP Media, Dr Sumit Gakkhar, Visiting Consultant – Psychiatry, Narayana Hospital, Jaipur explores what ARFID really is, why it happens, and how early understanding and support can make a big difference.

Can you explain Avoidant Restrictive Food Intake Disorder (ARFID) in detail and how does it affect children?

Avoidant Restrictive Food Intake Disorder (ARFID) is characterized by an individual’s avoidance or restriction of food intake, not driven by body image concerns, but rather by factors such as fear, discomfort, or heightened sensory sensitivities. Children diagnosed with ARFID frequently experience inadequate nutritional intake, which can result in fatigue, nutrient deficiencies, impaired growth, and broader health complications. Distinct from anorexia nervosa, the child’s motivation is not weighting loss or a desire for a thinner physique; instead, their relationship with food is influenced by apprehension, unease, or sensory overload.

How can parents differentiate between normal picky eating and ARFID?

Selective eating is frequently observed in young children and typically resolves by the age of five. In such cases, growth generally remains within normal parameters, and the child consumes a limited yet consistent selection of foods. Conversely, with Avoidant/Restrictive Food Intake Disorder (ARFID), the dietary limitations are sufficiently severe to impact the child’s growth, weight, nutritional status, and overall daily functioning. Children with ARFID may reject entire food groups, exhibit significant apprehension, or experience physical distress during meals. This behavior is persistent and does not spontaneously improve.

What psychological triggers commonly lead to ARFID?

Avoidant Restrictive Food Intake Disorder (ARFID) can be precipitated by several factors, including:

  • Phagophobia (fear of choking).
  • Emetophobia (fear of vomiting) or gastrointestinal discomfort.
  • Prior adverse or traumatic food-related experiences (e.g., a choking episode).
  • Sensory hypersensitivity, manifesting as a strong aversion to specific smells, textures, or flavors.
  • Allergic reactions to food or food-related medical emergencies.

For some children, ARFID may also co-occur with ADHD, autism spectrum disorder or anxiety disorders.

Parents should look out for:

  • Refusing meals or declining to join the dining table.
  • Experiencing extreme distress, crying, or panic when presented with new foods.
  • Reporting abdominal pain, nausea, or gagging during mealtimes.
  • Expressing fearful statements such as “The food will get stuck” or “I might vomit.”
  • Exhibiting an overwhelming response to food smell, color, or texture.
  • Consuming food at an exceptionally slow pace or taking very small bites.
  • Demonstrating limited appetite cues, with the child potentially stating they “never feel hungry.”

How early can ARFID develop? Does early childhood feeding stress play a role?

Avoidant/Restrictive Food Intake Disorder (ARFID) can manifest during early childhood. In certain instances, its onset may be abrupt, following a traumatic food-related event, such as choking or experiencing severe abdominal pain. Furthermore, early negative feeding experiences, including force-feeding, stringent mealtime regulations, or pressure to consume food, can heighten food-related anxiety and potentially contribute to the development of ARFID in some children.

If parents suspect their child has ARFID, whom should they consult first?

The initial step involves a consultation with a pediatrician to exclude any organic or medical etiologies contributing to the child’s feeding challenges. Should no medical concerns be identified, a referral may be made to:

  • A child psychologist or psychiatrist specializing in eating disorders.
  • A feeding therapist or occupational therapist proficient in sensory-based feeding issues.
  • A nutritionist with expertise in pediatric eating disorders, as deemed necessary.

What gentle, evidence-based therapies help children with ARFID or food anxiety?

Effective, child-friendly therapies include:

  • Cognitive Behavioural Therapy (CBT) is designed to assist children in confronting their fears, modifying anxious thought patterns, and fostering self-assurance concerning food.
  • Exposure therapy involves a gentle, progressive introduction to novel foods within a secure and encouraging setting.
  • Play-based feeding therapy employs enjoyable activities to alleviate anxiety and enhance sensory tolerance.
  • Family-based therapy provides guidance to parents on how to support their child without resorting to pressure or coercion.
  • Sensory integration therapy is available for children who experience sensory sensitivities, which are often observed in individuals with autism or ADHD.

What advice can help parents who feel helpless or guilty about their child’s eating struggles?

Parents should recognize that ARFID is a recognized medical condition, not a result of behavioral issues or parental shortcomings. Here are some helpful considerations:

  • Exhibit patience, as progress may be gradual but consistent.
  • Refrain from coercive feeding practices or undue pressure, as these can exacerbate anxiety.
  • Acknowledge and celebrate even minor achievements, such as a child’s willingness to smell or touch a new food.
  • Seek professional assistance promptly; parents are not expected to manage this challenge in isolation.
  • Prioritize their own emotional well-being. Parenting a child with ARFID can be demanding, and self-care is crucial for maintaining a supportive and composed demeanor.

ARFID is not a phase or a child “being stubborn”—it is a genuine challenge that affects a child’s health, emotions, and relationship with food. But with awareness, proper guidance, and compassionate support, children can learn to eat safely, confidently, and joyfully again.
