Surviving breast cancer is often seen as the end of a tough journey — but for many women, it’s the beginning of a new emotional battle. From fear of recurrence to body image struggles, the mental scars can linger long after treatment ends. To understand how psychological support can help survivors rebuild confidence and emotional well-being, THIP Media spoke to Dr Manisha Gaur, PhD in Clinical Psychology, Director of Gaur Mental Health Clinic, Ajmer.

What are the common psychological challenges faced by breast cancer survivors?

Many times, women feel that once the treatment is done, then everything will go back to normal. But in reality, life changes completely after that. And the journey doesn’t end with the last chemotherapy or radiation session. Emotionally and mentally, it can take a huge toll. One of the biggest fears is the fear of recurrence. Even a small body ache can trigger thoughts like, “Is the cancer back?” And this fear doesn’t really go away. Then there are body image issues—hair loss, scars, mastectomy—these can affect confidence and self-esteem. Some women go into depression, others face anxiety or even PTSD-like symptoms. It’s a lot to deal with.

What is a survivor’s guilt, and how does addressing it help breast cancer survivors?

Survivor’s guilt is when a woman feels guilty for surviving while others may not have. They think, “Why me? Why did I survive when someone else didn’t?” And this guilt can be really heavy. Talking about it, acknowledging it, and processing these emotions with a mental health professional or even within a support group can really help. It allows the survivor to shift from guilt to gratitude and start focusing on their own healing journey.

What helps survivors rebuild self-esteem and body image after mastectomy or reconstruction?

Rebuilding self-esteem starts with accepting the new body and being kind to oneself. Support from partners, family, and peers is very important. Some women find solace in joining support groups where others share similar experiences. Others may benefit from therapy, or engaging in activities that make them feel good about themselves—like dressing up, going out, or even participating in photo shoots that celebrate their journey. It’s about reclaiming identity and finding beauty in survival.

Do cultural and societal factors affect how breast cancer survivors cope?

Yes, absolutely. In some cultures, discussing cancer or mental health is still a taboo. There’s pressure to “be strong” or “not talk about it,” which makes it harder for survivors to express their emotions. They might feel isolated or judged. Even expectations around appearance—like looking normal or behaving like nothing happened—can be emotionally exhausting. So, creating awareness, having open conversations, and making mental health support culturally sensitive are really important steps.

How do mindfulness and stress reduction improve breast cancer survivors’ mental well-being?

Mindfulness and meditation help women stay in the present moment and reduce anxiety about the future—especially the fear of recurrence. Exercise, even something as simple as walking, boosts mood, improves sleep, and gives a sense of control over one’s body again.

What strategies can caregivers adopt to support the psychological recovery of breast cancer survivors?

Yes. Caregivers should listen without judgment. Try to understand what the woman is going through. Don’t just say “Be positive” or “It’s over now.” Acknowledge her pain, fears, and insecurities. Emotional support from family goes a long way in recovery.

How can returning to work positively influence the mental health of breast cancer survivors?

Returning to work gives a sense of normalcy, routine, and purpose. It makes the survivor feel productive and helps rebuild confidence. Of course, it should be done when the person feels ready—both physically and mentally. But yes, work can be a great motivator for recovery.

Do digital mental health platforms play any role in providing accessible psychological support?

Absolutely. Digital platforms can offer therapy, meditation tools, or even community support groups online. They make help more accessible, especially for those who can’t travel or live in remote areas. It’s convenient and reduces the stigma too, because you can get support from the privacy of your home.
