आंकड़ों से स्पष्ट है कि पाँच में से एक महिला को अपने जीवनकाल में एक बार UTI अवश्य होता है। रिकॉर्ड के अनुसार, पुरुष भी मूत्र मार्ग संक्रमण से पीड़ित होते हैं। इसके बावजूद, महिलाओं को उनके शरीर के शारीरिक कारकों, गर्भावस्था और मासिक धर्म के दौरान हार्मोनल परिवर्तन जैसे कारकों के कारण अधिक खतरा होता है। आइए महिलाओं में UTI को विषय से समझें और इसके लक्षणों पर भी चर्चा करें।

किन शारीरिक कारकों (anatomical factors) की वजह से महिलाओं में UTIs अधिक होता है?

UTI महिलाओं की मुख्य समस्याओं में से एक है, जिसका आमतौर पर एंटीबायोटिक दवाओं के साथ उपचार किया जा सकता है। डॉ. अनीता गुप्ता, एसोसिएट डायरेक्टर ऑफ ऑब्स्टेट्रिक्स एंड गायनेकोलॉजी, फोर्टिस ला फेम, नई दिल्ली बताती हैं “मूत्रमार्ग का आकार पुरुषों की तुलना में महिलाओं में छोटा होता है। दूसरा, यौन क्रिया भी मूत्रमार्ग को प्रभावित कर सकती है। तीसरा, योनि मूत्रमार्ग के निकट स्थित होती है। इस कारण, योनि संक्रमण से मूत्रमार्ग भी प्रभावित हो सकता है। इन सभी की वजह से महिलाओं में UTIs अधिक हो सकता है। बुजुर्ग महिलाओं में, म्यूकोसा, जो मूत्रमार्ग क्षेत्र की सबसे बाहरी परत है, थोड़ी पतली और सूखी होने लगती है, जिसके परिणामस्वरूप UTI का जोखिम भी हो सकता है।” इसके अलावा, रजोनिवृत्ति और मासिक धर्म के दौरान हार्मोनल परिवर्तन भी इसका कारण बन सकता है। इस दौरान, एक महिला के मूत्र मार्ग की रक्षा प्रणाली प्रभावित हो सकती है। इसके परिणामस्वरूप महिलाओं में UTI होने का जोखिम अधिक हो सकता है।

गर्भावस्था UTIs के खतरे को कैसे बढ़ाती है?

गर्भावस्था से एक महिला के शरीर में विभिन्न प्रकार के परिवर्तन हो सकते हैं। इसके अतिरिक्त, एक महिला का शरीर हार्मोनल, शारीरिक और भावनात्मक सहित कई बदलावों से गुजरता है। इन परिवर्तनों से मूत्र मार्ग संक्रमण का खतरा बढ़ सकता है। गर्भावस्था के दौरान महिलाओं में UTIs के मुख्य कारण निम्नलिखित हैं:

मूत्र मार्ग परिवर्तन

डॉ. रितु सेठी, MBBS, MD, स्त्री रोग और प्रसूति विज्ञान, एसोसिएट डायरेक्टर, मैक्स अस्पताल, गुड़गांव, हरियाणा बताती हैं, “गर्भावस्था के दौरान, एक महिला का गर्भाशय फैलता है, जिससे मूत्राशय पर अधिक दबाव पड़ सकता है। इसके परिणामस्वरूप, इसकी क्षमता कम हो सकती है, जो इसे पूरी तरह से खुलने से रोकती है। शेष पेशाब बैक्टीरिया के विकास के लिए एक अनुकूल वातावरण बना सकता है, जिससे UTIs का खतरा बढ़ जाता है। इसके अलावा, बढ़ता हुआ भ्रूण गर्भाशय और गुर्दे सहित सभी अंगों पर अतिरिक्त दबाव भी डाल सकता है।”

हार्मोन में परिवर्तन

गर्भावस्था से संबंधित हार्मोनल परिवर्तन मूत्र मार्ग की मांसपेशियों को आराम देते हैं, जिससे पेशाब प्रवाह धीमा हो जाता है। यह बैक्टीरिया के विकास को बढ़ा सकता है। इसके अलावा, प्रोजेस्टेरोन, महिलाओं में एक हार्मोन, मूत्रमार्ग और मूत्राशय को आराम दे सकता है, जिससे बैक्टीरिया के लिए मूत्राशय में पहुंचना आसान हो जाता है।

प्रतिरक्षा प्रणाली (Immune system) में परिवर्तन

गर्भावस्था माँ की प्रतिरक्षा प्रणाली को भी प्रभावित कर सकती है, जिससे शरीर के लिए संक्रमण से लड़ना कठिन हो सकता है। इसमें मूत्र मार्ग संक्रमण भी शामिल है।

स्वच्छता की कमी

स्वच्छता की कमी UTIs के फैलने का एक प्रमुख कारक है। इसके अलावा, लंबे समय तक पेशाब को रोकने, गलत दिशा में पोंछने और कम पानी पीने जैसी खराब स्वच्छता आदतें भी UTIs का कारण बन सकती हैं।

UTIs के सामान्य लक्षण क्या हैं जिन पर महिलाओं को ध्यान देना चाहिए?

डॉ. गुप्ता UTIs को मुख्य रूप से मूत्र के संक्रमण के रूप में परिभाषित करती हैं जो मूत्र मार्ग को प्रभावित कर सकता है, यह केवल प्रारंभिक स्थान में हो सकता है या इसके केवल लक्षण ही नज़र आ सकते हैं, लेकिन यह वास्तव में संक्रमण नहीं होता है। हालांकि, यह पुरुषों और महिलाओं दोनों में हो सकता है लेकिन इसके होने का खतरा महिलाओं में अधिक रहता है।

डॉ. गुप्ता महिलाओं में UTIs के लक्षण इस प्रकार स्पष्ट करती हैं, “महिलाओं में UTIs का मुख्य लक्षण पेशाब करते समय जलन होना है। इसके अतिरिक्त, आप बार-बार पेशाब करने की इच्छा होना भी इस संक्रमण का लक्षण हो सकता है। इसके अलावा, पेशाब का पूरी तरह से न होना या केवल एक-दो बूंद ही आना भी UTIs के लक्षण हो सकते हैं।”

UTI का कारण बनने वाले मुख्य बैक्टीरियों में से एक एस्चेरिचिया कोलाई है जिसे E. coli के रूप में भी जाना जाता है। हालांकि, यह आमतौर पर हमारी आंतों में स्थित होता है, जो हमारे लिए नुकसानदायक नहीं होता है लेकिन फिर भी यह UTI संक्रमण का कारण बन सकता है। ऐसा इसके मूत्र मार्ग जैसे शरीर के अन्य अंगों में प्रवेश करने की वजह से हो सकता है। ऊपर वर्णित शारीरिक कारकों के कारण, महिलाएं E. coli से संबंधित मूत्र मार्ग संक्रमण के प्रति अधिक संवेदनशील होती हैं।

महिलाओं में UTI होने की संभावना पुरुषों की तुलना में अधिक होती है। इसका मुख्य कारण पुरुषों और महिलाओं में मूत्रमार्ग के आकार में अंतर होना है। इसके अलावा, हार्मोनल परिवर्तन और गर्भावस्था भी महिला में UTI का कारण बन सकते हैं।
