Fact Check

What exactly is diabetes? How does it affect blood glucose regulation?

Diabetes is a metabolic illness characterised by progressive development over time. The medical condition is divided into two main categories:

  • T1D, also known as insulin-dependent diabetes, is an autoimmune condition that requires long-term insulin therapy because immune cells mistakenly attack the insulin-producing pancreatic beta cells.
  • Type 2, or adult-onset diabetes, is mostly caused by insulin resistance and chronic beta-cell dysfunction brought on by obesity, sedentary lifestyles, and heredity.

The body uses a process called blood glucose regulation, or glucose homeostasis, to keep blood sugar levels steady within a predetermined range, usually between 70 and 140 mg/dL. Maintaining this specific equilibrium is crucial to ensure a steady supply of energy for cells and to avoid the adverse outcomes of both high and low glucose levels. Diabetic dysfunction of this regulatory mechanism can cause blood glucose levels to increase or fall.

Can vinegar derived from jamun effectively cure diabetes?

No. While both jamun and vinegar have been implicated in the comprehensive management of diabetes, there is no robust scientific evidence to support the use of vinegar developed from jamun to manage diabetes.

Syzygium cumini, often referred to as ‘jamun’ or ‘black plum’, is packed with bioactive constituents such as iron, vitamin C, flavonoids, polyphenols, and antioxidants. This high bioactive profile may be the reason for its ability to regulate blood sugar levels and enhance insulin sensitivity. These results, however, remain confined to preliminary studies.

In a similar vein, vinegar is made via a two-step fermentation process and is a useful food element. When consumed with high-glycaemic meals, vinegar’s strong acetic acid content helps lower postprandial glucose and insulin responses and may somewhat increase satiety. Furthermore, mechanistic assessments point to benefits like better insulin sensitivity, regulation of hepatic glucose production, delayed stomach emptying, and enhanced glucose absorption.

Even while jamun and vinegar may exhibit antidiabetic effects individually, no randomised control trial currently evaluates the impact of jamun-derived vinegar for hyperglycaemia. Patients with diabetes should not depend on such unregulated alternatives for their treatment.

Are dietary alternatives to traditional hypoglycemics safe to use?

Diabetes management requires a carefully thought-out, personalised diet, but it cannot replace evidence-based medical therapeutics. A nutritious meal plan can help patients with T1D by supporting insulin therapy, and it can help those with type 2 by reducing their dependence on prescribed medication. While healthy dietary options may help, anti-diabetic therapeutics are still needed to attain optimal glycaemic control. Further casting doubt on their safety and effectiveness is the lack of clinical validation, standardised dosing, or quality assurance.

Diabetologist

Dr. Ritesh Bansal, the founder and senior consultant diabetologist of Delhi’s Saroj Diabetes & Research Centre, adds more credence to it by explaining that although dietary modifications can aid in the management of diabetes, they shouldn’t be regarded as a workable alternative to prescription diabetic medications. Therefore, alternative dietary options might be a beneficial addition that lessens the need for large dosages of medication. Ultimately, Dr. Bansal suggests consulting a professional before making any dietary adjustments as part of your therapeutic regimen.

Depending solely on unverified dietary substitutes may result in major issues, worsen the condition, and delay a precise evaluation. Therefore, even though some may be used as adjuncts, therapeutic measures strictly under medical supervision should always be employed to manage diabetes.

Can a persistent illness such as diabetes be completely cured?

No, not precisely. This is because a persistent illness is a long-term medical condition that may significantly impact day-to-day living and usually necessitates continuous care. These conditions frequently appear gradually and can last for years or even a lifetime.

It is imperative to clarify that, from a medical and legal standpoint, metabolic illnesses, such as diabetes, cannot be completely cured:

From a medical perspective:

T1D is presently considered incurable. Even though T2D patients may experience remission with significant lifestyle changes or medical intervention in its earliest phases, there is no permanent “cure” for the condition.

Dr. Naseemuddin N. Shaikh, Consultant Endocrinologist at Renova Century Hospitals in Banjara Hills, Hyderabad, clarifies the main difference between cure and remission. He further explains that the cure is when the illness is completely resolved and won’t come back, like when prescription medications are used for treating bacterial infections. Conversely, remission is the phase when the disease is under control and does not require treatment, albeit it may resurface.

This is especially true for type 1, which requires insulin therapy for the rest of one’s life. Early-stage type 2 diabetes may undergo remission with a comprehensive strategy that includes prescribed medications, low-glycaemic diets, and 30 to 45 minutes of aerobic activity each day. It is not advised to forego therapy because of unsubstantiated assertions made online.

The claim that diabetes can be cured is prohibited in India. This is because the Drugs and Magic Remedies (Objectionable Advertisements) Act of 1954 and Schedule J of the Drugs and Cosmetics Rules of 1945 both specifically forbid advertising for the treatment of diabetes, along with several other chronic illnesses. Therefore, people shouldn’t believe online claims that offer at-home or one-time solutions for diabetes.

What is the best course of action for diabetes?

As previously mentioned, hyperglycaemia calls for a multimodal approach. Uncontrolled blood glucose levels are usually managed using prescription medications, such as insulin therapy and oral hypoglycemics, along with a healthy lifestyle. This is further supported by timely patient education, psychological counselling, and monitoring of HbA1c levels every 3-6 months.

A key component of managing diabetes is preventing complications. If the prescribed medication is discontinued, blood glucose levels could rise, and the consequences of diabetes may worsen. The gradual rise could damage blood vessels and nerves, leading to cardiovascular illness, renal impairment, neuropathy, and vision loss.

Please understand that anyone battling hyperglycaemia in any form should refrain from employing any dubious online dietary options or remedies. The only way to successfully manage the chronic condition is with individualised, evidence-based care monitored closely by a qualified endocrinologist or diabetologist. Early and consistent intervention could lead to more favourable results and a higher standard of living.
