Why is it important to identify the type of kidney stone?

Identifying the type of kidney stone is important because different stones form for different reasons and require different treatment and prevention strategies. For example, certain uric-acid stones may be dissolved using medically supervised treatment that makes the urine more alkaline. However, calcium oxalate stones, the most common type, generally cannot be dissolved with medicines or home remedies.

Struvite stones are usually linked to urinary tract infections and may require antibiotics along with stone removal. Cystine stones develop because of a rare inherited condition and require specialised long-term treatment.

Doctors must also assess the stone’s size, location and whether it is obstructing urine flow. Small stones may pass naturally, whereas larger or obstructing stones may require shock-wave lithotripsy, ureteroscopy or surgery. Knowing the stone type also helps recommend appropriate dietary changes and medicines to prevent it from forming again.

Can lemon, honey and coconut water dissolve a large kidney stone?

No. None of these ingredients has been proven to break down large kidney stones.

Large stones are hard mineral deposits. They cannot be physically washed away merely by drinking more fluid. Extra fluid may increase urine production, but it does not necessarily shrink the stone or make a stone that is too large suddenly pass through the ureter.

The ureter is the narrow tube carrying urine from the kidney to the bladder. A large stone can become stuck in it, leading to severe pain, bleeding, infection or obstruction of urine flow. According to the National Institute of Diabetes and Digestive and Kidney Diseases, large or obstructing stones may require medical procedures such as shock-wave lithotripsy, ureteroscopy or percutaneous nephrolithotomy.

Therefore, this drink should not be promoted as an alternative to medical treatment.

What does “large kidney stone” mean?

The chance of a stone passing naturally generally decreases as its size increases. Location is also important: a stone close to the bladder may pass more easily than a similarly sized stone higher in the urinary tract.

Stones below approximately 5 mm often have a better chance of passing naturally. Stones between 5 and 10 mm may or may not pass and sometimes require medication or a procedure. Stones larger than 10 mm are less likely to pass without intervention.

For renal stones larger than 2 cm, the European Association of Urology recommends percutaneous nephrolithotomy as the primary treatment option in suitable patients.

These are general guides. A urologist must also consider the stone’s position, density, number, composition, symptoms and whether it is blocking urine.

What role does lemon juice play?

Lemon juice may help with the prevention of some stones, but that does not mean it can dissolve a large existing stone.

Lemon contains citrate. In the urine, citrate can:

  • Bind with calcium
  • Reduce the amount of calcium available to form crystals
  • Prevent small calcium crystals from joining together
  • Make it more difficult for some existing crystals to grow

Harita Adhvaryu, a senior dietitian and diabetes educator from Ahmedabad, explains that lemon juice may help reduce the risk of certain kidney stones, but it does not dissolve stones that have already formed.

Citrate may be particularly helpful for people with low urinary citrate who repeatedly develop calcium stones. However, lemon water is not the same as prescription potassium citrate. Potassium citrate provides a measured dose of alkali and is prescribed according to the patient’s stone type, kidney function, blood potassium and urine-test findings.

Some people would need to consume large quantities of lemon juice to obtain a therapeutic citrate dose. This may cause heartburn, stomach irritation or dental-enamel erosion. Diluting lemon juice and avoiding added sugar are sensible if it is being used as part of a prevention plan.

What role does coconut water play?

Coconut water can contribute to fluid intake, and adequate hydration is important in reducing kidney-stone risk. More dilute urine contains lower concentrations of stone-forming substances.

A small study found that coconut-water consumption increased urinary citrate, potassium and chloride in people without a history of kidney stones. However, it did not significantly increase urine volume compared with water or change urine pH. More importantly, the study did not show that coconut water could dissolve or remove existing kidney stones.

Plain water remains the main fluid recommended for stone prevention. Coconut water is not a medically established treatment and is not necessarily suitable for everyone.

Does honey help dissolve kidney stones?

No proven benefit has been demonstrated. Honey mainly adds sugar, sweetness and calories to the drink. There is no good-quality human evidence showing that honey can dissolve calcium oxalate, calcium phosphate, struvite, cystine or large uric-acid stones.

Adding honey does not improve the stone-dissolving ability of either lemon juice or coconut water. Excessive intake may also be unsuitable for people with diabetes, insulin resistance or obesity. Research has found an association between greater consumption of added sugars and kidney-stone prevalence, although this does not prove that a small amount of honey directly causes stones. If lemon water is being consumed for hydration, it is generally better to take it without added honey or sugar.

Can any kidney stone be dissolved without surgery?

Yes, but only certain stones and only with appropriate medical treatment.

Some uric-acid stones can be dissolved by making the urine more alkaline. Doctors may prescribe alkaline citrate or sodium bicarbonate and ask the patient to monitor urine pH. This approach is known as oral chemolysis.

The urine pH usually needs to be carefully maintained within a specific range. If it becomes too alkaline, the risk of calcium phosphate stones may rise. Therefore, treatment requires medical supervision, dosage adjustment and follow-up imaging.

The European Association of Urology guidelines state that uric-acid stones may be dissolved through medically supervised urine alkalinisation. This should not be confused with drinking lemon, honey and coconut water.

Calcium oxalate stones, which are the most common kidney stones, cannot generally be dissolved using food, drinks or currently available medicines.

Could drinking this mixture help a stone pass?

It cannot guarantee stone passage. Adequate fluid intake may support urine flow and help some small stones move through the urinary tract. However, drinking excessive amounts cannot force a large or obstructing stone through the ureter. It may worsen discomfort if urine is building up behind a blockage.

Doctors may prescribe an alpha-blocker, such as tamsulosin, to help selected ureteral stones pass. This is more likely to be considered for certain stones between approximately 5 and 10 mm, depending on their position and the patient’s condition. A suspected large stone should be assessed through appropriate imaging rather than treated only with fluids at home.

How are large kidney stones actually treated?

Kidney stone surgery treatment depends on stone size, location, composition, symptoms and complications. Options include:

  1. Shock-wave lithotripsy
    High-energy sound waves are used to break the stone into smaller fragments that may pass in the urine. More than one session may be required.
  2. Ureteroscopy
    A thin instrument is passed through the urinary tract. The stone can be removed or fragmented with a laser.
  3. Percutaneous nephrolithotomy
    A small opening is made through the back to access the kidney. This is commonly used for large, complex or multiple renal stones.
  4. Medical expulsive therapy
    Selected ureteral stones may be managed with medicines that relax the ureter and improve the chance of passage.
  5. Oral chemolysis
    Selected uric-acid stones may be dissolved by medically controlled urine alkalinisation.

So the said claim is false. Lemon juice may supply citrate, and coconut water may contribute to hydration, but neither has been proven to dissolve a large kidney stone.
