A well-shared Instagram reel is promoting a herbal replacement for root canal therapy. The post with the caption, “दांत में कीड़ा या RCT का डर? अब महंगे ट्रीटमेंट नहीं… ये घरेलू नुस्खा ही काफी है!”, emphasises that the combination of Iremedadi oil, turmeric, rock salt, and crushed cloves can effectively manage discomfort induced by dental caries. While using botanical extracts for managing dental discomfort is a frequent practice, it can be highly deceptive to think of these remedies as a substitute for RCT.

What exactly is root canal therapy? When is it necessary to have one?

Root canal therapy (RCT) is a dental intervention used for managing infection or inflammation in the dental pulp, which is the tooth’s innermost soft tissue that contains blood vessels and nerves. RCT is usually recommended when the pulp is irreversibly damaged due to:

  • Deep cavity development
  • Trauma
  • Repeated dental procedures
  • Cracks that could allow bacterial invasion

If timely intervention is not administered, pulpal infections can cause significant pain, bone loss, and abscess formation. During the procedure, the inflammatory or infected pulp is usually removed. After that, the canal or canals are meticulously cleaned and disinfected. After irrigation, the canal space is sealed with a biocompatible material to prevent reinfection. Recent advancements in endodontic techniques and imaging may further improve precision and success rates.

Therefore, root canal therapy is a restorative, tooth-saving treatment that reduces discomfort, aids in the removal of infection, and restores dental function, thus avoiding the need for extraction.

Do painful carious lesions respond well to Irimedadi oil combined with crushed cloves, turmeric, and rock salt?

No, not at all. Irimedadi oil, turmeric (curcumin), rock/sea salt, and crushed cloves are touted as an effective therapy for deep carious lesions; their relevance is confined to social media. Given that the ingredients are being considered for the most distressing dental pain, they should be carefully scrutinised.

Irimedadi or (Arimedadi) oil

We begin with Irimedadi taila, a common substance. This is an Ayurvedic medicinal oil that is predominantly employed for oral massage and rinsing as an oil-pulling option.  While Yashti, Trijatha, Manjishta, Gayatri, Lodhra, Katphala, Kshirivrikshatwak, Irimeda twak, Musta, Agaru, Shvetachandana, Rakta chandana, Karpoora, Jati, Tila taila, and other ingredients make up the preparation, its use has been assessed as an adjunct to mechanical plaque control, which is used for early gingival inflammation management and not for caries.

These conditions need to be managed by a dentist. Relying only on oil pulling could worsen the issue and delay the necessary therapy.

Rock Salt or Sendha Namak

Hence, neither of these ingredients can stop nor can they protect the tooth from further damage.

Should extensive carious lesions be dealt with uncertain remedies?

No, not exactly. This is due to the fact that tooth decay, also known as dental caries, is a progressive bacterial invasion. Irreversible pulpal damage may occur if the carious lesion extends to the dental pulp. If left untreated, it could result in severe discomfort, the development of an abscess, and even tooth loss.

Due to accessibility issues, budgetary limitations, and fear, individuals at risk utilise herbal oils, cloves, turmeric, or warm salt rinses. Although the procedure might temporarily alleviate symptoms, these web-based remedies are unable to penetrate the tooth’s deeper deteriorating layers and restore the necrosed dental tissue. Relying only on such unregulated alternatives could aggravate the infection, enhance the risk of systemic complications, and delay essential professional care.

Dependence on do-it-yourself fixes may cause irreversible damage and delay a professional assessment. As a result, every toothache condition needs to be evaluated by a qualified dentist.

How are caries handled by dental experts?

When treating painful cavities, dentists—especially endodontists—usually use a very meticulous approach. The initial step in the therapeutic approach is usually to identify the underlying cause and stage of infection. This is usually achieved by a thorough clinical and radiographic assessment that helps ascertain the state of the tooth pulp.

Cases that have progressed to irreversible pulpitis or pulpal necrosis necessitate root canal treatment. This timely removal of the infected tissue can prevent abscess formation or systemic spread. Hence, this Irimedadi-based herbal formulation may be a useful adjunct but should not be a replacement for endodontic treatment for irreversible pulpal damage.
